Most companies fail at innovation before they even start. Not because they lack ideas or tools, but because their organization is not built to support innovation. After assessing innovation readiness at 40+ industrial companies over 25 years, I keep seeing the same eight mistakes. Not tool problems. Organizational problems.

Most companies that fail at innovation do not fail because of bad ideas. They fail because the organization was never ready to support innovation in the first place.

I have assessed innovation readiness at over 40 industrial and manufacturing companies in 25 years. Chemical producers, equipment manufacturers, B2B suppliers, industrial services companies. The pattern is consistent: about half of them had already invested in innovation tools, workshops, and programs before I arrived. And about half of those investments had produced nothing.

Not because the tools were wrong. Not because the teams were lazy. Because the organizational conditions for innovation were broken, and nobody had diagnosed them honestly.

Innovation readiness mistakes are different from framework mistakes. They happen at the meta-level: the process of assessing and building your organization’s capacity to innovate. Get this wrong, and it does not matter which canvas you use or how many experiments you run. The organization will kill your innovation efforts before they produce results.

Here are the eight mistakes I keep seeing. And what to do instead.

Mistake 1: Assessing readiness with a survey instead of observing behavior

This is the mistake that produces the most misleading results.

A company sends out an innovation culture survey. Leadership scores high on “we support experimentation.” Middle management scores high on “we are open to new ideas.” The survey says the organization is ready to innovate.

Then a team actually tries to run an experiment. They need €5.000 for a customer test. The approval process takes three months, four sign-offs, and a business case that requires three-year revenue projections for something that has not been validated yet. The team gives up. The experiment never runs.

The survey lied. Not because people were dishonest, but because surveys measure intentions, not behavior. People genuinely believe they support innovation. Until someone actually asks for budget, time, or permission to fail.

I saw this at a packaging manufacturer. Their annual employee survey showed 82% “innovation readiness.” When I asked the leadership team how many experiments their teams had actually run in the past 12 months, the room went silent. The answer was two. For an organization of 800 people with an official innovation strategy.

What to do instead: Assess readiness by looking at what people actually do, not what they say. How many experiments ran last quarter? How fast can a team get approval for a small test? What happened the last time a project failed? Who got blamed? The gap between survey scores and observable behavior is where your real readiness problems live.


Mistake 2: Confusing innovation theater with innovation culture

Innovation theater looks like innovation culture from the outside. There are innovation days. Hackathons. A wall of sticky notes in the lobby. A Chief Innovation Officer with a LinkedIn profile full of keynote photos. An innovation lab with beanbag chairs and a 3D printer nobody uses.

None of this is innovation culture.

Innovation culture is what happens when someone proposes an idea that threatens the core business. When a team asks for resources to test something unproven. When an experiment fails and the question is: “what did we learn?” versus “whose fault is this?”

A manufacturing company I worked with had all the theater. Innovation days, internal pitch competitions, a dedicated space. But when a product team proposed testing a subscription model for their equipment, the sales director killed it in one meeting. “That would cannibalize our service contracts.” No data. No testing. Just a gut reaction from someone protecting existing revenue.

That is the real test of innovation culture. Not what happens on innovation day. What happens on a regular Tuesday when someone challenges the status quo.

What to do instead: Ignore the visible signals. Ask three diagnostic questions: What happens when an innovation project threatens core revenue? What happens when an experiment fails? Who has the authority to say yes to an unproven idea without escalating? If the answers reveal protection, blame, and bureaucracy, you have theater. Not culture.


Mistake 3: Assuming leadership buy-in because the CEO gave a speech

The CEO stands on stage at the annual kickoff. “Innovation is our number one priority this year.” Applause. Slides about disruption. A new innovation budget line in the annual plan. Everyone leaves the room believing leadership is committed.

Six months later, the innovation budget gets quietly reallocated to a core business project that is behind schedule. The innovation team’s headcount request sits in HR limbo. The quarterly review focuses entirely on core business metrics. Innovation is not on the agenda.

The speech was real. The commitment was not.

Leadership buy-in is not a speech. It is not a budget line. It is not a strategy document. Leadership buy-in shows up in three places: where time goes (does leadership attend innovation reviews?), where money stays (does the innovation budget survive the first reallocation pressure?), and what gets rewarded (do people who run experiments get promoted, or do they get sidelined?).

I have seen this pattern at at least 15 companies. The CEO genuinely believes in innovation. But the operating model, the reward structure, and the meeting cadence all optimize for the core business. Innovation gets the words. The core business gets the behavior.

What to do instead: Stop measuring leadership commitment by what leaders say. Measure it by three things: Do they attend innovation portfolio reviews? Has the innovation budget survived reallocation pressure? Has anyone been promoted or recognized specifically for innovation work in the past year? If the answer to all three is no, you do not have leadership buy-in. You have a speech.


Mistake 4: Treating readiness as a one-time checkbox

“We did an innovation readiness assessment last year. We’re good.”

No. You assessed a snapshot of your organization at one point in time. Since then, three senior leaders have changed, a reorganization shifted reporting lines, the market shifted, and the innovation team lost two people.

Innovation readiness is not a state you achieve. It is a capability you maintain. Like physical fitness: you do not run a marathon once and declare yourself permanently fit. The organizational conditions that support innovation require ongoing attention.

A chemical company I worked with ran a solid readiness assessment in Q1 of one year. By Q3, a new CFO had arrived and introduced a zero-based budgeting process that required every innovation project to justify its existence from scratch every quarter. The readiness assessment from six months earlier was meaningless. The organizational conditions had fundamentally changed.

What to do instead: Build readiness assessment into your regular rhythm. Quarterly is realistic for most organizations. Track the same dimensions over time: leadership engagement, budget stability, decision speed, experiment throughput, failure response. The trend matters more than the score. A company scoring 3 out of 5 but trending upward is in a better position than a company scoring 4 but declining.


Mistake 5: Building an innovation lab without changing incentives

The innovation lab is the most expensive mistake on this list.

A company invests €500.000 in a dedicated innovation space. Hires a team. Gives them freedom to explore. The lab produces prototypes, concept videos, and pitch decks. None of it goes anywhere.

The reason is predictable: the lab operates in a bubble. The people in the lab are incentivized to explore. Everyone else in the organization is incentivized to exploit. When a lab project needs engineering resources from the core business, the core business says no. When a lab idea needs a pilot customer, sales says “not my quota.” When a lab concept needs manufacturing capacity for a small test run, operations says “we are at 94% utilization, come back next quarter.”

The lab did not fail. The incentive system around it guaranteed failure.

What to do instead: Before building any innovation lab or team, answer one question: what happens when innovation needs something from the core business? If the answer is “they go through the normal process,” you are setting up the lab to fail. Innovation needs protected resources, dedicated time from key people (not “when they have bandwidth”), and explicit leadership backing to pull resources when needed. Mistake 6: Hiring an innovation manager without giving them authority

This is the organizational equivalent of asking someone to drive a car with no steering wheel.

A company creates an “Innovation Manager” role. The job description sounds exciting: develop innovation strategy, build innovation pipeline, foster innovation culture. The title looks good. The mandate looks clear.

But the Innovation Manager reports to the Head of R&D, who reports to the COO, who cares about operational efficiency. The Innovation Manager has no budget authority (everything above €10.000 needs CFO approval). No hiring authority. No ability to redirect engineering time. No seat at the leadership table where strategy decisions happen.

The Innovation Manager becomes an event organizer. They run workshops, manage the idea box, and produce monthly innovation reports that nobody reads. They leave after 18 months, frustrated. The company concludes that “we tried innovation management and it didn’t work.”

It did not work because the role had responsibility without authority. This is a setup for failure in any function, but it is especially destructive in innovation because innovation requires crossing organizational boundaries that the manager has no power to cross.

What to do instead: If you hire an innovation manager, give them three things: budget authority for small experiments (at least €25.000 per quarter without additional approval), direct access to leadership (a seat at the table, not a monthly report), and the explicit right to pull people from other functions for time-boxed projects. If you cannot give those three things, do not hire the role. You will waste the person’s time and your money.


Mistake 7: Copying Silicon Valley playbooks into industrial B2B

This mistake has cost more money than all the others combined.

A leadership team reads a book about how Google runs innovation. Or attends a conference where a startup founder explains lean methodology. They come back inspired. “We need to be more like a startup. Move fast. Fail fast. Disrupt ourselves.”

Then they try to apply it. In a company that makes industrial equipment with 18-month sales cycles. Where a failed experiment does not mean a bad landing page, it means a €200.000 prototype that does not meet safety certification. Where “talking to customers” requires six months of relationship building and an NDA. Where “pivoting” means retooling a production line.

Silicon Valley playbooks assume short feedback loops, low experiment costs, digital products, and direct customer access. Manufacturing and industrial companies operate with the opposite of all four. Long feedback loops. High experiment costs. Physical products. Intermediated customer access through OEMs, distributors, and procurement departments.

The methodology is not wrong. The translation is wrong. A Business Model Canvas session works for an industrial company, but the follow-up looks completely different. A Value Proposition Canvas works, but the jobs, pains, and gains look nothing like a SaaS company’s. Testing business ideas works, but the experiments are different: technical feasibility studies instead of landing pages, pilot installations instead of beta apps, co-development agreements with key accounts instead of online surveys.

What to do instead: Adapt the principles, not the playbook. The principle of evidence-based decision-making applies everywhere. The principle of testing before investing applies everywhere. But how you test, what “fast” means, and what experiments cost will be completely different in an industrial context. Find a practitioner who has actually worked with manufacturing companies, not someone who read about it in a startup book.


Mistake 8: Measuring readiness by inputs instead of outputs

“We allocated €2.000.000 to innovation this year. We hired 12 people for the innovation team. We bought three software tools. We are clearly committed to innovation.”

None of that tells you whether innovation is actually happening.

Budget is an input. Headcount is an input. Tools are inputs. They measure what you invested, not what you produced. A company can spend €2.000.000 on innovation and produce zero validated business models. Another company can spend €100.000 and validate three new revenue streams.

The outputs that matter: How many business ideas were tested with real customers this quarter? How many assumptions were validated or disproven? How many projects were killed based on evidence, not opinion? How many ideas moved from exploration to a real pilot? How quickly can a team go from idea to first customer evidence?

I worked with an industrial services company that tracked their innovation progress exclusively by budget spent and projects in the pipeline. Their dashboard looked impressive: 14 projects, €1.800.000 invested, 23 team members involved. When I asked how many of those 14 projects had any validated customer evidence, the answer was one. The other 13 were running on assumptions.

That is not an innovation portfolio. That is an assumption portfolio. And managing a portfolio without evidence is just organized guessing.

What to do instead: Track four output metrics: experiments run per quarter, assumptions validated per project, kill rate (what percentage of projects were stopped based on evidence), and time-to-first-evidence (how many weeks from idea to first real customer data). These metrics tell you whether your organization is actually innovating, not just spending money on innovation.

The operating model of a successful company is optimized for efficiency, predictability, and risk reduction. Those are not bugs. They are the reason the company is successful.
